Elder Care Professionals Referrals

Referral from a Home Care Agency:

Situation: A 76-year-old woman, caring for her chronically ill 84-year-old husband in their home, had a home care agency providing services for two hours a day following his discharge from the hospital. The home care case manager noticed that the patient’s wife was becoming increasingly overwhelmed with the responsibility and expressed concern over how long she could continue. The woman also had financial questions regarding what would happen if her husband took an extended stay in a nursing home.

The case manager had attended one of our CEU Presentations regarding long term care planning and, with the wife’s permission, called us during a home visit to discuss the situation.

Referral from a Skilled Nursing Facility:

Situation: A Discharge Planner from a Rehab Center/Skilled Nursing Facility was working on a discharge plan with a widowed 82-year-old recent stroke patient and her family. The family’s goal was to bring the patient home, but the case manager had concerns about their ability to care for her in the home and advised them that she would need care 24 hours a day/7 days a week. Knowing that the cost for this care would be higher than if she were to stay in the facility, the case manager encouraged the family to contact SRC in order to discuss how to preserve her assets, as well as possibly qualify for Medicaid Benefits in the Community.
